Responsibilities

As an Education Nurse you will:

 

Responsible for teaching/mentoring and modeling the Centering Pregnancy (CP) philosophy with new co-facilitators. Maintains group spreadsheets to include current enrollment, demographic data,contact information, and pertinent medical/obstetrical data. Accounts for all medical records and reviews charts for: currency, accuracy/precision of individual patient input, transcription of laboratory data. Ensures availability of a designated space for conducting the Centering Pregnancy®sessions as well as room preparation for each session that includes setting up according to location and session, obtaining appropriate audio/visual supplies and equipment, gathering/replenishing teaching materials, and reorganizing supplied equipment. Assists as needed with self-assessment

activities including blood pressure rechecks, coordination of appropriate lab collection, answering questions, providing referrals, planning group activities with facilitator, facilitating social supportsystem, promoting partnership between provider/facilitator and group members, and empowering leadership within the group. Identifies, schedules, and makes follow-up correspondence with guest speakers, when necessary. Responsible for overseeing the logistics of care to the patients in the designated population; convening the steering committee for regular meetings; and executing decisions of the steering committee. Communicates and collaborates with Women’s Health Program Administrator (WHPA) to schedule on-site system redesign visit with the Model Implementation PlanConsultant (MIPC).

Coordinates through facilitation in the implementation of a group prenatal care and health education program for the active duty and beneficiary population at volume eligible (MTF's).Interacts with clinical staff, administrative officers, and other members of the multi-disciplinary health care team. Based on assessments, Department of Defense (DoD), and Department of Army(DA) priorities, plans, and coordinates health risk interventions. Collects and analyzes the beneficiary population data (including patient demographics, medical utilization patterns, and outcome data) to determine prevention and disease/condition management program needs.Develops primary, secondary, and tertiary intervention programs using the most current science and research in consultation with subject matter experts. Analyzes the health care needs of a defined population using Centering Counts Summary Tools. Develops sessions based on the resultsof assessment surveys and other data with guidance from the /co-facilitator. Provides analytical support duties including database maintenance. Compiles and reports on health education data after every cohort.

Facilitates effective patient education and utilization of health care resources. Co-facilitiates inthe implementation of educational sessions that address a wide variety of conditions for helping patients achieve and maintain healthier lifestyles. Collaborates with provider to develop unique activities and materials necessary to meet the patients’ needs. Schedules patient education ingroup settings. Continually reviews and refines program and associated instructions based on initiative’s guidelines. Is a member of a multidisciplinary team/committee in support of the clinical practice guidelines. Collaborates with clinical departments and services throughout facility to identify educational requirements needed to enhance abilities of healthcare support personnel to promote, accomplish, and reinforce patient education. Collaborates in planning, coordinating, and implementing education and training activities.

Plans, coordinates, and schedules session dates, providers, and facilitators. Assigns patients to appropriate groups. Markets and publicizes the Centering Pregnancy® initiative through coordination with Army Community Service, providing information in family practice and obstetric clinics at theMTF, and promotion outside of the facility through newspaper advertisements and hospital fairs.Provides administrative support for health promotion programs. Facilitates the completion of patient surveys at the post-partum reunion. Coordinates administrative activities with medical support assistants. Conducts reminder and follow-up calls, locates charts, follows-up on all missed sessions,and schedules individual appointments as needed.

Qualifications

US Citizenship required

 

A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation ofNurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. 

 

A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
