Vance Air Force Base, Oklahoma is located in Garfield County, on the south side of the City of Enid. Enid is approximately 95 miles northwest of Oklahoma City, 115 miles west of Tulsa, and 123 miles south of Wichita, Kansas. This is a military town and the community appreciates our presence.
Enid, Oklahoma, (population 48,000) is located in northwestern Oklahoma and is the county seat of Garfield County. The community was founded in 1893 by means of a land run during the Cherokee Outlet opening. Enid was once an oil industry boomtown and home to Champlin Petroleum. The city is also supported by a nearby military installation; Vance Air Force Base was created four miles from Enid in 1941. Enid boasts more than 30 parks and recreation areas. One of them, Government Springs Park, was originally a watering hole on the Old Chisholm Cattle Trail.

POSITION DUTIES:
The Nurse will serve as a clinical case manager to provide timely, quality, comprehensive, cost effective healthcare resource utilization and care support for our Department of Defense (DoD) beneficiaries with complex care needs. This is accomplished through clinical case management, care coordination and discharge/disposition planning utilizing military and/or civilian (purchased care) healthcare systems and resources.
1. Develops, implements, and monitors the MTF’s clinical case management program for inpatient and/or ambulatory care settings, in accordance with DoD and TRICARE Management Activity (TMA) guidelines. Develops local clinical case management protocols and procedures to facilitate quality care, military and/or civilian(purchased care) healthcare coordination and collaboration, maximize MTF resources, and improve patient outcomes while managing overall MTF costs. Facilitates collaborative clinical case management processes, which assesses plans, coordinates, implements, monitors, and evaluates care options and medical resources/services, to meet complex health needs across the care continuum with the goals of decreasing fragmentation of care and enhancing the patient’s quality of life while containing costs. Employs a holistic approach considering diagnosis, prognosis, pathophysiology, physical, mental, psychological, and emotional needs, demographics, and home environment to provide the optimal level of care and patient outcome. Clinical case management screening includes, but is not limited to: individuals with acute, chronic, multiple, complex, catastrophic, life threatening illnesses/injuries and/or high medical services utilization rates; patients that are non-compliant with their care regimen; and those patients who warrant extended services following discharge. Coordinates with Managed Care Contractor for clinical case management issues when beneficiaries require the majority of their care outside the MTF. Coordinates services and benefits with civilian facilities as needed. Serves as a part of an interdisciplinary healthcare team to ensure appropriate coordination of care to enhance patients’ health and wellness, safety, productivity, and quality of life. Develops and implements individualized clinical case management care plans. Utilizes DoD, agency, TMA and local MTF directives for data collection techniques. When required, coordinates Military Health System (MHS) care with the Department of Veterans Affairs (DVA), other DoD medical facilities, settings, inter or intra-TRICARE regional transfers, and transfer of cases between Commands or between facilities (for instance from one hospital to another, from one level of care to another level of care, or from overseas to stateside to facilitate continuity of care).
2. Provides robust and comprehensive clinical case management, care coordination, and discharge/disposition services. Assesses and identifies appropriate patient care needs by evaluating high-risk cases, high-utilizers, intensity and complexity of services, health care costs, and non-compliance issues. Coordinates care and collaboration between multiple military and/or civilian (purchased care) healthcare agencies. Ensures a seamless transfer of information to facilitate continuity in the patient’s plan of care. In collaboration with the patient, medical professionals, interdisciplinary care team(s), and TRICARE, develops patient-specific high quality individualized care plans to achieve cost effective outcomes on an ongoing basis. Proactively identifies and evaluates patients and families for clinical case management/care coordination from a variety of sources such as discharge/disposition planning, referrals, the Medical Evaluation Board (MEB) process, interdisciplinary team meetings, etc. Educates the patient and family on the clinical case management process, healthcare and treatment options, community resources, insurance benefits, and psychosocial concerns so informed decisions can be made. Supports the provider/patient relationship and plan of care, utilizing evidence-based practice guidelines and patient empowerment strategies to reduce/prevent exacerbations and complications. Attends mandated training in accordance with DoD, agency, TMA and local MTF guidelines. This includes but not limited to, any required Case Management/Medical Management conferences for certification/competency, as well as, monthly clinical case management teleconferences.
3. Evaluates clinical case management activities and conducts ongoing monitoring and evaluation of the Clinical Case Management Program using objective criteria. Provides a holistic approach to case management, care coordination, and discharge/disposition services that includes: diagnosis, prognosis, pathophysiology, physical, mental, psychological and emotional needs, demographics, and the home environment to provide the optimal level of care and patient outcome. Reviews/analyzes medical records, and other associated documents to determine quality and appropriateness of clinical care requirements, cost of care, and to modify care plans, as well as, facilitate adherence to practice guidelines, standards, and/or procedures. Collaborates with interdisciplinary team(s), as needed to ensure that appropriate clinical case management activities are being met, i.e., care coordination, discharge/disposition needs, and/or medical equipment needs/services. Additional monitoring and evaluation may include, but is not limited to, patient’s adherence and response to treatment plan, identification of variables, patterns or trends from established practice guidelines and/or standards, established outcome measurements, results of interventions, treatment delivery and timeliness of care, and utilization of resources. Maintains documentation and data collection in accordance with DoD, agency, TMA, and local MTF guidelines.
4. Collaborates/coordinates with non-MTF and community healthcare facilities and services in support of care continuity. Such facilities and services include, but are not limited to: Managed Care Support Contractor, Department of Veterans Affairs (DVA), Social Service agencies, Home Health and Community Health Service, community hospitals and rehabilitation facilities, and companies with durable medical equipment (DME), etc. Implements clinical case management, care coordination, and discharge/disposition activities to match patients’ needs to available resources within the MTF and community. Coordinates with TRICARE and/or Third Party Payer in the selection of facility/resource; taking into account individual health care coverage. Ensures proper authorization is processed appropriately. Identifies and prioritizes patients’ request for alternative services and/or alternative level of care. Arranges or assist in arranging, DME, as necessary, to fulfill discharge requirements in absence of discharge planner. Utilizes DME benefits through familiarity with local and national vendors, researching such issues as reasonable and customary costs, medical necessity, purchase versus rental decisions, maintenance contracts, and consideration of back-up equipment for life sustaining equipment, such as ventilators. Organizes and updates internal and community resources to provide effective patient management, promote quality outcomes, and reduce costs.

Basic Requirement for Nurse:
Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: Graduates of foreign nursing schools must have completed the Commission on Graduates of Foreign Nursing Schools Certification Program. The certification program consists of a credentials evaluation, the CGFNS Qualifying Exam, and evidence of passing one of the approved English Language Proficiency examinations. A copy of the CGFNS Certificate is required.
Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before.
