Lyster Army Health Clinic  is located at the home of Army Aviation on Fort Rucker, Alabama, and serves about 17,500 patients, including permanent-party Soldiers, Soldiers attending short-term schools, foreign Soldiers, family members, and retirees and their family members.

 

Lyster has embraced the Patient-Centered Medical Home model throughout the clinic and recently consolidated its active duty flight status Soldiers into one clinic for better continuity of care. Lyster has 14 clinics (including three primary care teams) on the first floor and loans the second floor space to the Wiregrass VA.  Patients needing specialty care, such as surgeries, or consultations not provided within the clinics, are referred to the surrounding network. Providers work closely with network specialists to ensure their patients receive the best care possible.

Responsibilities

As a Physician (Internal Medicine) you will:

 

Serves as Internal Medicine Physician. Scope of services include, but are not limited to, examination, diagnosis, prevention and treatment of adults, usually 18 years of age or older. Care provided includes a broad spectrum of health services ranging from preventative health care to treatment of acute and chronic diseases. Provides long-term comprehensive care managing both common and complex illnesses. Primary care internal medicine incorporates an understanding of disease prevention, wellness, substance abuse, mental health and common health problems. Takes ownership, responsibility for care of assigned panel of patients. Provides first contact, continuous, comprehensive primary care for adults according to current standards of care, the patient centered medical home model of care, Army policy, and relevant regulations. Leads a multi-disciplinary team of individuals who collectively takes responsibility for the ongoing care of patients. Coordinates and/or integrates care across all elements of the healthcare system (e.g.,
subspecialty care, hospitals, home health agencies, nursing homes) and the patient's community (e.g., family, public and private community). Completes required records (both paper and electronic) pertaining to all medical services provided in accordance with specified procedures and regulations. Properly safeguards all reports and records according to the Health Information Portability and Accountability Act (HIPAA) standards. Ensures the highest standards of professional practice and ethics are maintained. Teaches and provides consultation to other health care providers as assigned. Participates in Continuing Medical Education (CME) and quality assurance activities as assigned.

Qualifications

US Citizenship Required

 

Board Certified Internal Medicine

 

 

Doctor of Medicine or Doctor of Osteopathy from a school in the United States or Canada approved by a recognized accrediting body in the year of the applicants graduation. [A Doctor of Medicine or equivalent degree from a foreign medical school that provided education and medical knowledge substantially equivalent to accredited schools in the United States may be demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates (ECFMG) (or a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country).

 

Must have a permanent, full, and unrestricted license to practice medicine in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
